Currently, we refer to files, mostly elixirs, inconsistently. Sometimes, we render it as code, sometimes not and in some cases underscores are not rendered correctly. I unified the links to always render as code (so it is not necessary to escape underscores). Additionally, I added links to some references to elixirs and fixed other rendering issues.
