Several bug fixes to get library functional (before, failing to import), including:

Upgrading qiskit (qiskit 0.38.0 does not support the CCX gate)
Remove non-existent imports from algorithms/vqe
CU updates from #165
[WIP] Commented out gates that fail the CI/CD — will review and fix them (possibly in this PR or in another)
Misc. updates and reformatting
