Summary
As the "Book us" button on the home page and "Book appointment" buttons on the repairs page lead to the same slot booking form, we can make it more specific by adding "repairs" as a form input by a drop-down menu so that it will be straightforward for the admins on which aspect the customer is talking about.
Fixes #261
Added the inputs type of service and type of repair and added a few changes.
