Multiple annotations for an example within a campaign

Currently, the crowdsourcing campaign expects that each example will be annotated exactly once.

We should add an option to specify the maximum number of annotators for each example (>=1).

This needs to be reflected both in db.csv (where the example will need to be listed multiple times) and in the browse interface which currently expects only one annotation per output within a campaign.
