Create a backbone view to display address information. Address information also contains phone information.

Template should include:

checkbox to select row
Address Type
Primary Address status
- If the address is primary, it should say "Primary"
- If the address is not the primary address, it should say "Make primary" on mouse over of the row
Address info
- street1
- street2
- city, state/province country zip
Phone info
- phone1
- phone2
- fax
