This happened when the tool was run against project which was not built (packages not restored). Is this by desing? Reporting usage of package multiple major versions above what is actually used? Would it not be better to show error that package was not found?

This is a shortcoming of the algorithm used in the released version. Please try the prerelease version which will use the same algorithm the nuget command uses. You should no longer see such issues
I just released https://www.nuget.org/packages/nuget-license. This is a release of my fork of v3.0.0 of this tool. Unfortunately @tomchavakis has been unresponsive for quite some time and the nuget api key expired which means I was unable to release in the original form.
