image preview just show no image icon

Hi All,
my kcfinder can browse well on images
but when i put in text box area, it just show no image icon in the preview and no image in text area
some issues related with httaccess, folder access had i tried. but no result.
for insert flash and file work fine.
could you describe in detail what should i do?which is the wrong one?
any support will be appreciate.
