Everything on the spine needs to have a corresponding toc entry

It is an unfortunate situation that all items on the spine require a corresponding toc entry on the current LWP. That means that you can't have pure content files that have no title in them.

Fixing this will not be that easy because at the moment the toc is assembled directly from looping the spine. The paradigm I opted while developing LWP was that a file equals a chapter. Clearly, I was mistaken.
