get error when running in kubenetes v1.28.6

when click "save" button after entering all data (name of user, template, namespace), an error dialog appears and immediately disappeared :
"Permission manager can't retrieve the user's permissions. This is caused by a manual action on the cluster. try to recreate the permissions"
=> but the user was created, i still can authenticate, authorize the new created user. dont know why.

i deploy permission-manager by helm chart (v1.9.0),
