Provide crashdump (if any)
State any changes made to the plugin that wasn't pushed to GitHub
Provide plugin version or commit
State version of PM and if using a spoon
Provide screenshots (if applicable)

If you haven't already noticed, most orphans are spammed with issues which are either spam or don't provide enough information.
