Minimal test case: -

Plone 4.3.11
Enable ToC feature on default home page..
plone.app.contenttypes 1.1.1
Upgrade to dexterity content types. All is well at this step.
plone.app.widgets 1.9.1
After installing the Dexterity content types all is well, but after installing plone.app.widgets 1.9.1, the ToC is gone.

The odd thing is that I see toc.js in the JavaScript registry, but it does not show up in the page.
