Change instances of:

<xsl:attribute name="data-pdf-bookmark" select="@data-pdf-bookmark"/>

To:

<xsl:attribute name="data-pdf-bookmark">
<xsl:value-of select="@data-pdf-bookmark"/>
</xsl:attribute>

For broader XSLT processor compatibilty
