Additional handling needed around footnotes for TOC/XREF generation #162

Special handling for footnotes for these two cases:

TOC generation such that when a TOC entry is generated for a heading containing a footnote, footnote is suppressed from TOC entry.
XREF generation such that when an XREF points to an element that contains a footnote, footnote is suppressed from gentext.
