unrecognized configuration parameter "lock_timeout" #305

Tried running the ledger with PostgreSQL 9.2 and got an exception

app CRIT error: SET lock_timeout = 0 - unrecognized configuration parameter "lock_timeout"

Seems like lock_timeout has been introduced in PostgreSQL 9.3. This needs to be fixed or documented that we support PostgreSQL 9.3+
