In the design the initiator of an Extended Key Update message will update only its sending key. Hence, if you and I send an Extended Key Update at the same time the result will be that both our sending keys will be updated. This will result in new application traffic secrets in both directions.
What if both endpoints initiate a key update at the same time? How do you determine which update is the one that produces the next key?
