Is your feature request related to a problem? Please describe.

The Issue Template github uses is in .github/ISSUE_TEMPLATES and still references the CONTRIBUTING.md guidelines of fluent-plugin-elasticsearch.

The ISSUE_TEMPLATE.md file in the root of the project is not recognised by github.

Describe the solution you'd like

Cleanup old ISSUE_TEMPLATE, move new ISSUE_TEMPLATE from ./ to .github/ISSUE_TEMPLATES/

Describe alternatives you've considered

Keeping it as it is ;-)
