various broken links and broken formatting

In glancing through https://fido-alliance.github.io/how-to-fido/HowToFIDO.html just now, I notice that there's at least two links within the doc that 404:

1. "account chooser-style UI" -- https://www.accountchooser.com/learnmore.html (the resource is gone)
2. in footnote 6 "Signing-in with a UVRA" -- https://fido-alliance.github.io/how-to-fido/53-signing-in-with-a-uvra -- it should be: https://fido-alliance.github.io/how-to-fido/HowToFIDO.html#53-signing-in-with-a-uvra ( this may have been my mistake in pr editorial fixups 1 #34 )
3. footnote 13: "below" should link to https://fido-alliance.github.io/how-to-fido/HowToFIDO.html#53-signing-in-with-a-uvra ..? but the term "below" is out-of-context

Also, the content of the table in section 1 Overview is broken -- the HTML for the links to footnotes is visible to the reader. I thought I had this done correctly in PR #34, something with this doc has changed since the latter PR landed?
