Display either package upload or manual entry of launch url and activity id

When creating a new tincanlaunch activity, it can be confusing to have both the package upload and the manual entry (launch URL and activity id). This should be simplified similarly to how mod_scorm has implemented external vs local packages.

It can also cause issues when the tincan.xml activity id differs from the one provided in the manual entry.
