As a user, I want to state if I smoke or vape to indicate my risk level for pulmonary complications, especially if I contract COVID-19.

Acceptance Criteria

It's done when I am prompted with a question asking about whether I smoke or vape.
It's done when there's a list of options and I can only select one.
It's done when there's a back button available in case a user wants to change their previous answer.
