As a user, I want to state any pre-existing conditions to communicate my risk level of handling COVID-19 if contracted.

Acceptance Criteria

It's done when I am prompted with a question asking my pre-existing health conditions.
It's done when there's a list of checkbox options and I can select more than one.
It's done when there's a back button available in case a user wants to change their previous answer.
It's done when there's a Next button that allows me to navigate to the next screen when done and I've selected at least one option.
