As a user, I want to state if I think I've had COVID-19 but have not been able to confirm it.

Acceptance Criteria

It's done when I am prompted with a question asking if I think I had COVID-19 but have not been tested.
It's done when a list of responses are viewable and and only one can be selected.
It's done when there's a back button available in case a user wants to change their previous answer.
