It's possible that these already exist and I just don't know where to find them, but we should have a section added to the handbook about feedback forms for the workshops. I made individual ones for each subject, using this as a template: https://docs.google.com/forms/d/1KA958UZGEGbYTIFz9LIqjCVB8cJFUHBZ6xCT3TmNoNs/edit. In the top right corner next to the username, there are three dots, and if you click those, one of the options is "make a copy". So I could use that and then rename it something relevant and then edit the questions as I wanted. All I did was delete the first question, since I was making separate ones for each subject. Then you can get a link to share with the learners by clicking the big purple "save" button when you're done with the edits.
