Build front end skeleton for subscription management page

  • Page implements designs here
  • Page is located at /subscribers/<id>/subscriptions
  • When page loads, checkboxes should be pre-populated with subscriber's current subscriptions
  • Accordions and CD checkboxes should be disabled/enabled following same requirements in Subscribe page front end skeleton #1542
