  • Display the judgements
  • Inform users that judgements will be reset if they reset their identity
  • Inform users that after submission, they must post their GM address in #on-chain-identity to be verified
  • add all native fields
  • make display name and discord address required fields

Should probably also display a red shield in case the judgement is one of the negative options
