USAXS has one of these with EPICS support and screens.

In our case we use only 8 out of 12 filters, but number of filters should not really matter. Primary need for my use is to be able to insert Attenuation index value which we will have predefined in our instrument settings table for specific operations of instrument. The PV name is : 12idePyFilter:Filter1:sortedIndex Other Pvs are kind of optional as we do not need them for operations.
This is what we were using for PF4 filters except we had two individual banks, here we have only one bank of all filters together. More convenient.
