Provide a sample grafana chart for the prometheus metrics. Something simple that works with multiple nodeos instances and shows all the metrics supported.

@matthewdarwin Hi Matthew, prometheus and grafana aren't our area of expertise. If you provide a config for a chart we will add it to our leap repo in the appropriate place.

Ok, will need to wait until I test the new code, which will be 5.0.0-rc1 or later.
